We are seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Home Health Nurse. The primary responsibility of this role will be to provide high-quality, personalized care to patients in their homes. The ideal candidate will have excellent clinical skills, a strong sense of empathy, and the ability to work independently.

Registered Nurse (RN) Responsibilities:

Conduct thorough assessments of patients’ health status and create individualized care plans
Administer medications and treatments as prescribed by physicians
Monitor patients’ progress and update care plans as needed
Educate patients and their families on health management and self-care techniques
Coordinate care with other healthcare providers, such as physicians, pharmacists, and physical therapists
Provide emotional support to patients and their families during challenging times
Document all care provided and communicate effectively with the healthcare team
Registered Nurse (RN) Skills and Qualifications:

Valid Registered Nurse license in the state of practice
Minimum of 1 year of nursing experience, preferably in home health or a related field
Strong clinical skills and the ability to make independent judgments
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced and ever-changing environment
BLS certification required, ACLS certification preferred

Registered Nurse (RN) Working conditions:
This role requires frequent travel to patients’ homes, so a valid driver’s license and reliable transportation are necessary. The ability to lift and transport equipment and assist patients with mobility is also required.
