Responsibilities of the Licensed Occupational Therapist:
Evaluate patients functional abilities and develop individualized care plans to promote independence and safety.
Provide skilled therapy interventions in diverse settings, including SNF, short-term rehab, assisted and independent living, and home health environments.
Implement treatment plans focused on improving patients’ strength, mobility, fine motor skills, and activities of daily living (ADLs).
Educate patients, caregivers, and family members on adaptive techniques and equipment to support long-term functional gains.
Collaborate with interdisciplinary team members, including PTs, STs, nursing staff, and case managers, to ensure cohesive and effective patient care.
Maintain accurate and timely documentation in compliance with regulatory and company standards.
Conduct home safety assessments and recommend modifications to enhance the patients home environment.
Monitor patient progress, adjusting treatment goals as needed to achieve optimal outcomes.

Qualifications of a Certified Occupational Therapy Assistant (COTA)
Valid OT License in the State of Ohio
