> REF:#032f1d54ad4d5bc1 *Charge Nurse for Subacute/Rehab Unit* plays a critical leadership role on a busy rehab and postacute care unit and is responsible for overseeing patient care, nursing assistants, and ensuring compliance with healthcare regulations. This position requires an organized nurse who can assess patient needs, coordinate care plans, and provide guidance to unit staff in a fast-paced environment. The Charge Nurse will work closely with physicians and other healthcare professionals to deliver high-quality care to assure with the team that patients will go home stable and cleared.

Experience of 3 years in LTC preferred as a Licensed Practical Nurse in long term care preferably. Seeking strong, organized, and flexible LPN who can work and communicate well with medical staff, residents and families as it relates to day to day resident care and services. Works with RN Subacute Unit Manager and Nursing Administration to assure residents needs are met timely and care plans and orders are carried out. Monitors the activities of CNAs and completion of tasks and assignments. Responsible for treatments, appointments, physician visits and assures shift documentation and charting are maintained and completed. Maintains unit compliance with facility and regulatory policies and procedures. Makes rounds on unit.
